Attendance questionnaire

Hi everyone. I am a 3rd year student at Liverpool John Moores University. I am currently in the process of conducting a study into the factors that affect attendances at EPL matches, which includes the use of a questionnaire. I would greatly appreciate any questionnaire responses that people can give. The questionnaire should take no longer than 5-10 minutes to complete and enquires into the motivations and considerations of supporters when deciding to attend football matches.
