Bolton Wanderers are offering supporters the chance to win the following prizes simply by completing a questionnaire.

- Two hospitality places in the Wanderers Suite for the club's upcoming fixture with Sunderland on 7 May
- One men and women's toiletry products hamper
- A selection of men and women's toiletry products

The club would like supporters to give opinions about shopping habits. The questionnaire shouldn't take more than five minutes to complete.

Fans need to enter their details at the end of the questionnaire to have a chance of winning these prizes.

The questionnaire and prize draw closes on Monday 2 May.
