Research for Interaction Design Module - you guys can help!

Hello everyone, I'm in my third year of web design degree and I have a module entitled 'interaction design'.

For my assignment, I have to gather data to form requirements - we have to pick a product and research into how people make the 'jump' to buy it.

My product is 'your favourite team's football shirt', so it would be fantastic and a great help if people could fill this out.

Completed. Seemed to be a problem with the 'store or web' section - it would only accept 'web' for an answer. Could have had an any other comments box too.

Thanks clapton, I'm currently correcting everything - I've never used wufoo before and I seem to be battling against parts of it.
