On planet hoboh ideas are a kind of giant pin ball machine pinging here, there and everywhere, making funny noises as they go... and he's not yet hit stage one replay...

And, sadly, never will, since it requires co-ordination between brain and the two fingers necessary to post on T-W, and that doesn't come easy...

So instead we have the racket between one mad thought and the next, before he sinks onto the pile of old newspaper, opens the bottle of strongbow, and contemplates - slowly - the next intervention...

Who are you talking about? GPs? Because the proposal is that they run the whole show, not just their own practices. And they're not very keen, on the whole, they don't like the idea. They've not been running it before.

Are you saying that you do or don't agree with GP's running the whole thing?

Ok lets go then,

GP Practices are far better places than they used to be offering vastly superior "local" in house treatment and are looking to expand these in a good many cases, who runs them now quite effeciently? Doctors! I can see if they are to be finacially capped why some will get on their high horse but unless you I and everyone else wants to stump up 90% of our earnings in tax they have to have budgets and be run on business lines.

hmmm... that's the spin... and to some people it sounds good - GPs running healthcare - cos we like GPs, don't we..

however....

Bish lets not even bother painting the worst case scenario, lets just look at the immediate, certain future under these proposals.

Imagine if Mr Hoboh walks into his GP surgery now with an infection in his ear.

Mr GP currently decides what is best for Mr Hoboh and either, prescribes something or refers him to see a specialist.

Lets say he decides a prescription may clear it up but as its recurring and a bit of a problem and potentially nice Mr Hoboh could lose his hearing in the worst case. So its worth a referral to a specialist.

CURRENTLY Mr GP refers him and doesn't think anything more of it, until he gets results back or sees Mr Hoboh again.

However, in a year or two's time according to Tory plans, Mr GP will be sitting on a local consortium who will have to manage the health budget for the area Mr Hoboh lives in.

So replay the situation, Mr GP's consortium are struggling to reign in their budget because a lot of Mr Hoboh's friends have had a bit too much to drink and have all needed to go into hospital within the last 6 months for various minor but very costly operations.

Mr Hoboh goes in with his ear problem and Mr GP who is worried about Mr Hoboh but also has one mind on the consortium he fronts up and manages and the budgetary problems he has. So when it comes down to a choice between a relatively cheap prescription or a more expensive referral to a specialist which hat will Mr GP have on, the one that provides the best possible care for Mr Hoboh or the one that protects his interests within his consortium and their financial position (that will no doubt incur heavy penalties from the department of health if they overshoot their budget)?

Does anyone think putting GP's in that position is a good idea in any way at all?

Most of the GP's bloody don't. They want to treat sick people not worry about budgets, services and management!

You're obviously missing the bit where the GP could offer an enhanced service to Mr Hoboh if he has medical insurance or is willing to contribute a small fee...

Can anyone confirm the clock on Daybreak was wrong this morning!? (arrgh busted watching daybreak:oops: )

But seriously, I lay in bed for about 10 mins waiting for the clock to strike 7am as my cue to get up. It did, so I hopped out of bed, dressed and out the door.

Thought it was unusual how bright it was for 7.30am, and strange there were so many people knocking about – when I landed at work and the place was full I knew summat was wrong, so checked my watch and sure enough it was 8.40am! I wanna know if the clock was wrong or maybe I was just dreaming or summat – was very weird.

Sounds like you were watching the new ITV1+1 channel by mistake. If you haven't retuned your box yet, it will have replaced ITV2+1.

I had no idea who she is. I can't watch DayBreak. Not coz my telly's broken or anything, I just can't watch it. I developed an allergy to ITV's morning news magazine programme during the Fiona Phillips days & I'm not willing to risk seeing if it's better now that Bleakley's on it. I very much doubt it though.
